Salon Suite Disinfection Service Agreement
This Service Agreement (the “Agreement”) is entered into between PureShield Electro-Disinfecting (“Provider”) and the Client identified below (“Client”) for recurring or per-visit electrostatic disinfection services performed at the Client’s salon suite. Client & Suite Information
Scope of Services
Provider will perform professional electrostatic disinfection of the Client’s salon suite using EPA-registered PURTABS disinfectant applied with Protexus cordless electrostatic sprayers. Each service includes:
- 360° electrostatic application to high-touch and client-contact surfaces — styling chairs, stations, counters, mirrors, wash basins, treatment tables, and reception areas.
- Disinfection of restroom and shared common-area touchpoints within the suite.
- EPA-registered, NSF D2 food-contact-safe, bleach-free solution applied at manufacturer-tested dwell times.
- A proof-of-service confirmation the Client may display for patrons and state board inspectors.
Services are a supplement to, and not a replacement for, the Client’s own routine cleaning and state-board-required sanitation and implement disinfection practices.
Suite Size & Pricing
Select the suite size and billing option that applies. Pricing is locked in for the term of this Agreement.
Term, Scheduling & Payment
- Term. This Agreement begins on the Effective Date below and continues on a month-to-month basis until cancelled per Section 5 — or, for the best value, on a 12-month term as shown below.
- Scheduling. Recurring services are performed on Friday, Saturday, and Sunday, on a mutually agreed day, unless an emergency weekday visit is scheduled upon availability.
- Payment. Weekly plans are billed weekly; per-visit services are due upon completion. Accepted methods: credit/debit, digital wallet, Cash App, Zelle, Venmo, and cash (exact amount only).
- Late Payment. Invoices unpaid within seven (7) days may pause service until the balance is current.
Commit to a 12-month term and lock in a discounted flat monthly rate — a savings of up to $60 per month versus standard weekly billing.
Cancellation & Rescheduling
- Either party may cancel this Agreement with fourteen (14) days’ written notice (email accepted).
- Individual visits may be rescheduled with at least 24 hours’ notice at no charge; same-day cancellations may incur up to 50% of the visit rate.
- No long-term commitment is required — recurring service continues only while both parties agree.
General Terms
- Access. Client will provide safe, timely access to the suite at the scheduled service time.
- Safety & Compliance. Provider uses EPA-registered products and OSHA-compliant, ETL-certified equipment, applied by trained technicians following label dwell times.
- Liability. Provider maintains general liability insurance. Provider is not liable for pre-existing damage, items not disclosed prior to service, or surfaces sensitive to moisture that were not identified by Client.
- Personal Property. Client should secure or remove sensitive electronics, documents, and open product prior to each visit.
- Entire Agreement. This document is the entire agreement between the parties and supersedes prior discussions. Texas law governs.
